Day Habilitation Services Provider

Essential Functions and Scope of Responsibilities: 

The Day Habilitation Services Provider has the primary responsibility to provide community-based/out-of-home support and assistance in achieving goals related to social activities and life skills. Day Habilitation services must be provided outside of the home. The job location for this service is out in the community. Only the client may be transported in the vehicle with the DSP. 

Job Duties: 

• Accurately document and track the progress of goals;
• Supervise/assist client in tracking medications and health care needs;
• Must have use of a reliable vehicle;
• Possess the ability to perform all tasks and activities as needed by this service;
• Manage finances for the client with proper documentation. 

Rate of Pay: 

$26/hr.
